Upon enrollment, the following original credentials should be presented:

1. First Year
1. BS Pharmacy and BS Medical Technology
1. Form 138 (High School Card) having the notation Graduated or Eligible for College with a minimum average of 80 percent and at least 80 percent grade in Math, Science and Physics;
2. Certificate of Good Moral Character from High School graduated;
3. 4 pcs. ID picture (1x1)
4. Placement test result
5. Alien Certificate of Registration
2. Certificate as Pharmacy Techinician
1. Form 138 (High School Card) with the notation Graduated or Eligible for College with a minimum general average of 80 percent and at least a final grade of 78 percent in Math, Science and Physics;
2. Certificate of Good Moral Character from the High School graduated;
3. 4 pcs. ID pictures (1 x 1)
4. Placement test result
2. Transferees (All courses, FIRST and SECOND YEARS ONLY)
1. Certificate of Honorable Dismissal;
2. Transcript of Records of previous College or its equivalent with no mark of FAILURE, INCOMPLETE, DROPPED OR WITHDRAWN and with a general average of at least 80 percent (2.5) in all subjects credited to the course;
3. Certificate of Good Moral Character from the school last attended;
4. 4 pcs. ID pictures (1 x 1)
